In order for your health claim to be considered for reimbursement, you must complete and sign this claim form. Please mail or fax this completed claim form with itemized bills and receipts to the address or fax number listed below, or scan and send via the CIGNA website. Please tape small receipts on 8.5 x 11 inch or ISO A4 paper. Please do not staple receipts to claim form.

Georgetown University provides you access to dental coverage through CIGNA International while you are employed through the SFS-Q campus in Doha. Employee pre-tax premium contributions are required. Dental coverage can only be changed during open enrollment, or as a result of a qualifying family status change.
In April 2008, CIGNA International announced the availability of the CIGNA Dental network discounts for our members in the U.S. with CIGNA Dental coverage. For more information, click here.

Flexible Spending Accounts (FSA)
FSAs allow employees to set aside money (pre-tax) to pay for eligible health care and dependent care expenses. You must enroll, or re-enroll, in FSAs each year. The enrollment period coincides with Open Enrollment for the Washington, DC campus of Georgetown Univeristy (November 1 – 30).

Georgetown University has contracted with CIGNA Behavioral Health to provides SFS-Q faculty and staff with access to services designed to assist with a variety of professional and personal concerns.
No employee premium contributions are required.
This program provides:
• Telephonic counseling (unlimited)
• 3 face-to-face visits with professional counselor
• Additional work/life support benefits, as needed

Mariana Barrancos
Mariana Barrancos has a Masters in Social Work from New York University (NYU, 2004) and a Psychology degree from Universidad Catolica Argentina (1991). She is a Licensed Psychologist in Qatar and Argentina and a Licensed Clinical Social Worker in Rhode Island, US. During her graduate studies Mariana Barrancos received the Bilingual Bicultural Scholarship from NYU and was a Glass Center Mental Health Research Fellow at NYU School of Social Work.
She has practiced as a Psychologist in Argentina in governmental agencies and in private practice. Her institutional work focused on the prevention and treatment of multiple forms of abuse, trauma and emotional deprivation in children, adolescents and adults. In the US she worked as a Licensed Clinical Social Worker in outpatient mental health clinics. Her areas of expertise include adjustment disorders, anxiety, depression, trauma and cultural diversity.
Dr. Patricia M. Bonewits, Ph.D.

Dr. Bonewits has over 25 years experience providing clinical and psycho-educational interventions. She hold licenses in her native state of Texas and in California. Her experience is varied working with individuals including the following:
- individuals from varied cultures and backgrounds
- school age youngsters and adolescents
- adults with career issues, including career transitions
- families adjusting to changes
- people in crisis following any traumatic life event, including post-traumatic stress
- individuals adjusting to changes in health, physical and other changes
As a Clinical Psychologist, Dr. Bonewits specializes in providing intervention and relief from the effects of varied clinical problems that are causing emotional pain or discomfort, for example:
- adjustment to change
- anxiety
- stress and coping
- depression
- personal crisis
- family issues affecting self or members of the family

Salary Continuance
Salary continuance begins after an eligible employee is unable to work due to illness or injury.
The Plan pays 100% of monthly earnings. Staff must satisfy a 15-day waiting period before becoming eligible to receive benefits. Benefits continue until:
– You are no longer disabled;
– You reach the “recovery limit” for your type of disability; or
– You reach the Maximum Allowable benefit period of 60 days.
No employee contributions are required for this benefit.

Long Term Disability Insurance
CIGNA long term disability insurance provides income protection if you are disabled due to injury or sickness.
Employees pay the full cost of this coverage on an after-tax basis.

Basic Life and AD&D Insurance
Life insurance can provide security for your beneficiaries in the event of your death or terminal illness. Enrollment for Basic Life and AD&D is automatic.
No employee contributions are required for this benefit.
Effective January 1, 2008
Basic Life Insurance coverage is equal to a benefit of $20,000 through MetLife Insurance Company.
Basic AD&D Insurance coverage also provides a $20,000 benefit. This benefit is payable in the event of death or significant loss due to an accident. Examples of significant loss include loss of sight and dismemberment.

Additional Life Insurance Options
You may elect or increase your supplemental life, voluntary AD&D and dependent life coverage between October 15 - November 15, 2007 through Employee Access+. Your elections will take effect January 1, 2008.

Supplemental Life
Effective January 1, 2008, MetLife will be the new life insurance carrier for Georgetown University. Life and accident insurance can provide financial security for your beneficiaries in the event of your terminal illness or death. The Supplemental Life Insurance Plan is a voluntary, employee-paid group term life insurance plan in which you can choose to participate to supplement your Georgetown-provided life insurance. Enhanced features offered by MetLife include free will preparation service. For more information about this benefit, click here.
Voluntary AD&D
Georgetown University provides a Basic Accidental Death & Dismemberment (AD&D) benefit of $20,000 for eligible faculty and staff. You may choose to purchase additional AD&D coverage for yourself and your family through MetLife. Effective January 1, 2008, Voluntary AD&D pays benefits in the event of death due to accident, but also pays in the event of a covered disabling injury. The proceeds from the AD&D coverage can help replace lost income and lessen the impact of costs associated with serious injuries, including medical bills, physical therapy, psychological counseling, safety upgrades to a home or car, and more.
